In this edition we are announcing two new sections of the journal which will be made available in subsequent volumes of VHJOE. The first concerns an International section chaired by Dr Manoop Bhutani. As briefly described in his announcement, Dr Bhutani is an internationally recognized gastroenterologist with a long list of accomplishments. Dr Bhutani frequently travels to all of the populated continents to lecture and participate in forums. Over the years he has developed close working relationships with outstanding gastroenterology colleagues around the world, and was a natural fit in this endeavor. All of us at VHJOE are very pleased that Dr Bhutani has taken on this important role, and we expect it will expand and improve our journal.

The second new section involves capsule endoscopy. This growing area is somewhat unique to modern gastroenterology. Usually, the endoscopist is present in a hands-on procedure. Capsule endoscopy reading takes place away from the patient, and in that sense resembles the medical practice of a radiologist. However, in contrast to radiology, the capsule reader is often asked to manage the patient based on the image, and this often requires a decision on whether or not to go to surgery for what could be an incidental finding. For example, the image below is from a case of an elderly woman presenting with undiagnosed blood loss anemia. Several small non-bleeding arteriovenous malformations were identified as well as the lesion shown deep within the jejunum. Should this elderly woman go to surgery or balloon endoscopy? What if this lesion was arterial, and at endoscopy biopsy had uncontrolled bleeding. Hemorrhage from small bowel nodules have been described.
